Medical Spanish for healthcare providers: past medical history
1. ¿Tiene algún problema crónico, como asma o diabetes?
(Do you/she/he have any chronic problem, such as asthma or diabetes?)
2. ¿Alguna vez ha sido hospitalizado?
(Have you/she/he ever been hospitalized?) Note: Literally, of course, this says, "Any time, have you/she/he been hospitalized?"
3. ¿Alguna vez, ha tenido una conmoción cerebral?
(Have you/she/he ever had a concussion?)
4. ¿Ha tenido problemas en el corazón?
(Have you/she/he had problems with the heart?) Note: Observe the translation of the word "en" above. Prepositions are very interchangeable when translating from Spanish to English.
5. ¿Alguna vez, le han hecho cirugía?
(Have you ever had surgery?) Note: Literally, this says, "Any time, have they done surgery on you?"
6. ¿Le han puesto todas las vacunas indicadas?
(Have they given you/him/her all the indicated vaccines?)
6a. ¿Están sus vacunas al día?
(Are your/his/her immunizations up to date?)
7. ¿Cuándo fue la última vez que recibió la vacuna antitetánica?
(When was the last time that you/he/she received the tetanus vaccine?)
8. ¿Ha sido atendido en la sala de emergencia frecuentemente?
(Have you/he/she been attended to in the emergency room frequently?)
9. ¿Ha tenido hipertensión arterial?
(Have you/he/she had arterial hypertension?)
10. ¿En el pasado, le han hecho chequeos médicos de rutina?
(In the past, have you had routine medical check-ups?) Note: Literally, this says, "have they done to you routine medical checks?"
11. ¿Le han hecho la mamografía anual?
(Have they done your/her annual mammography? Literally: "Have they done to you/her the annual mammography?")
12. ¿Le hacen la citología anual?
(Do they do to you/her the annual cytology (pap smear)?)
13. ¿Le han hecho la prueba de piel para tuberculosis, el PPD?
(Have they done to you/him/her the skin test for tuberculosis, the PPD?)
14. ¿Cuántos embarazos anteriores ha tenido?
(How many prior pregnancies have you/she had?)
15. ¿Tuvo algún problema con algún embarazo anterior?
(Did you/she have any problem with any prior pregnancy?)
