1. ¿Ha tenido problemas con la vista?
(Have you had vision problems?) Note: Keep in mind that the translation, "you," can be replaced here with "he" or "she." This also holds for the rest of the following review of systems questions below, unless it is gender specific content.
2. ¿Se ha sentido mareado?
(Have you felt dizzy?)
3. ¿Ha perdido peso recientemente?
(Have you lost weight recently?)
4. ¿Ha tenido vómitos o diarreas frecuentemente?
(Have you been vomiting or having diarrhea frequently?) Note: In Spanish, it is said literally, "Have you had vomits or diarrheas frequently?"
5. ¿Le ha dolido la cabeza últimamente?
(Has your head hurt you recently?) Note: Note the use of present perfect tense, "ha dolido," or "has hurt."
6. ¿Cuándo fue su último período?
(When was your last period?)
7. ¿Su período viene todos los meses?
(Does your period come every month?)
8. ¿Presenta alguna molestia antes o durante el período?
(Occurs any symptom before or during the period?)
9. ¿Le parece que ve doble o borroso?
(Does it seem to you that you see double or blurry?)
10. ¿Le da dolor en el pecho o se siente cansado después de algún esfuerzo?
(Do you have chest pain or do you feel tired after some effort?)
11. ¿Tiene dolor en las piernas si camina durante unos minutos?
(Do you have pain in the legs if you walk for a few minutes?)
12. ¿Es Usted sexualmente activo?
(Are you sexually active?)
13. ¿Tiene alguna pareja fija?
(Do you have a steady partner?)
14. ¿Usa protección durante las relaciones sexuales?
(Do you use protection during sexual relations?)
15. ¿Le ha aumentado el apetito o está comiendo más de lo normal?
(Has your appetite increased or are you eating more than normal?)

(Are you having more thirst, or are you drinking more water than before?)
17. ¿Ha notado cambios en la piel, como manchas o alguna otra lesión?
(Have you noted changes in the skin, like spots or some other lesion?)

(Have your nails become brittle?)

(Do you drop things easily?)

(Do you go to the bathroom regularly?)

(How frequently do you have a bowel movement?)
23. ¿Se está parando más para orinar en las noches?
(Are you getting up more at night to urinate?)
24. ¿Presenta dolor o ardor cuando orina?
(Do you have pain or burning when you urinate?) Note: "Presentar" in this case literally means "present, show, exhibit, display," etc.
